What is the purpose of the Edmund Harms Foundation
Organize care for the cancer patients in the final phase of life as well as possible at home in order to make that period as comfortable as possible, so that the patient can say goodbye to loved ones and loved ones in peace and quiet in their own familiar environment.
Difference from palliative and terminal home care:
Palliative home care: used when no improvement is possible in a specific situation. In this case no more cure for cancer patients.
In short, the palliative phase is the omen of the terminal phase. (can take half a year, a year or more).
Terminal home care: when the life stage is expected by the doctor at 3 months. or less is estimated. A characteristic of this is that the “real” medical care (to cure the disease) is being discontinued.
The EHF focuses on terminal home care.
The care is aimed at giving patients and their loved ones the highest possible quality of life. This is done through soothing and relief from suffering.
This includes the prevention and treatment of physical pain, but also the remedy of problems that can arise on the social, psychological or spiritual level. The care at home is provided by professional employees who can therefore also relieve the caregiver.
If it is decided to organize EHF in the home situation, certain conditions apply, namely:
• The treating physician must assume medical responsibility.
• Caregivers must be able to supervise the patient in situations. when no professional help is present / available. The patient may therefore not be home alone.
• Want to receive care in the home situation.
• Life expectancy of 3 months. or less.
Procedure for admission of patients to the EHF
• As soon as the patient is in possession of a letter stating that he / she has finished treatment and has a life expectancy of 3 months. or less (terminal phase) they can contact us.
• Intake interview will be scheduled as soon as possible. (preferably at home even when the patient is still admitted, as we can see the home situation and plan care better).
• During the intake, mutual acquaintance and expectations are discussed.
• Aids such as bed, wheelchair and primary chair will be arranged after the intake interview (delivery time 1 to 2 working days).
